Necessary Medical Forms and Information

All students, including new and returning students entering grades four and seven, are required to undergo a physical examination before attending school in California. Additionally, it is mandatory to have up-to-date immunization records on file with the front office prior to attending class in accordance with California State requirements. To ensure proper enrollment, all necessary medical forms must be downloaded from the student registration/enrollment portal (RenWeb/Facts) before attending classes.
