Health Examination and Immunization Requirements for School Entry

California School Immunization Law, Health and Safety Code Sections 120325-120375 requires that parents provide a copy of the vaccination report for each child, and that all required vaccinations are up to date and on file before the child begins classes. In addition, students are required to have a Report of Health Examination for School Entry form completed by a healthcare provider  before entering school for the first time, within 18 months of entering 1st grade, and before entering 4th and 7th grades.
